HOW TO COMPLETE THE ORDER FORM

1) Fill out the first part of the form with your name and address information.You must put the same address that your credit card bills are sent to for the billing address. In the next section put where you want the products to be sent to, if it is different than the billing address.

2) Decide which method of payment you prefer.

3) If paying with credit card, put your card # and expiration date. If sending by US mail or fax please sign and date the form, if by e-mail, put all information as it appears on the credit card. For credit card orders, you must remember to put the the billing address that your credit card bills are sent to.

4) Include all items you wish to purchase and make sure you fill out all of the information completely.

5) Texas residents need to add 6.25% tax. Residents of Denton County need to add 7.75%. If you are outside Texas, you do not need to add tax.

SHIPPING INFORMATION

Shipping within continental US is $4.95, regardless of the weight of the order.  MUST BE A UPS DELIVERABLE ADDRESS!!Shipping within Texas is $3.95 to a UPS deliverable address.International customers, Hawaii, Alaska, APO/FPO customers, and PO BOX addresses please email for shipping quote.       

Orders Outside the Continental USA

We welcome international orders! We have shipped worldwide and we look forward to more orders from other countries!Only credit card orders are accepted.In addition to the normal shipping charges, you agree to pay all additional shipping, handling, customs, and insurance costs. Due to cost, we only ship US Parcel Post. You may choose either surface or air. Surface takes 6-10 weeks, air takes 1-2 weeks. Air is more expensive.

International RegulationsAs a customer, you are responsible to comply with International, National or local laws regulatingimportation of products that you may purchase.  If an order sent by us to you is refused delivery by customsdue to unauthorized ingredients or contents, Muscle Warehouse is not to be heldresponsible for any losses or costs incurred by the customer.  Retrieval of any such shipment from customs is at theoption of Muscle Warehouse.  Muscle Warehouse will abandonany shipment when the cost of that return is more than the declared product value.  If the shipment is abandoned by Muscle Warehouse, the customer will not receive a refund or credit of any kind.If in any case we accept return of any shipment refused by customs authorities of any country,Muscle Warehouse will credit the customer for the value of the products purchasedless all shipping charges incurred for the return of the shipment and all shipping charges sending the product.There is usually not an importation problem, but occasionally they occur.By ordering, you agree to be bound by these terms.

CONDITION OF SALE

When ordering we reserve the right to hold any shipment prior to bank clearance of any credit cards or Company or Personal Checks.

Muscle Warehouse is not responsible for any typographical errors.

All orders are subject to shipping and handling fees.

COD ORDERS

We no longer accept COD orders due to the high rate of returned shipments.

RETURNS

If for some reason you do not receive the products that you want or just do not want them, send them back for a complete refund. All we ask is that you pay to have them sent back. The products must be unopened, sealed, and returned within 30 days of the invoice date. You must contact us for a RETURN AUTHORIZATION NUMBER or we will not credit your account. Without this number please do not send it back. We had to start doing this because people were taking advantage of the system. The original invoice must also be included. By placing an order you agree to these terms.
